We use a small number of vendors to host this website and deliver email notifications. These vendors are permitted to process information only to provide services to us and are required to safeguard it appropriately.

You may choose not to submit information through this website and instead contact the practice by phone. You may ask us to correct or delete information you submitted through the site by contacting us using the details below, subject to any records we are required to retain.

We provide care to children and adolescents, and appointment requests for a minor should be submitted by a parent or legal guardian. This website is not designed for independent use by children.
